Auto alert (notification) if source goes offline, relogin,etc

Thinking it would be useful to be able to create an alert notification that tells you if a source goes offline, relogs in or is stuck in a specific state like logging in. Occasionally I find a source hung up but don't realize it till I go to the sources tab in the dashboard.

we show notification if MT detects faulting automatically

also relogin automatically

if MT is not doing that, it won't detect for an alert either
